Registration Instructions

The Information / Forms area contains the Grade K – 8 Registration Packet for the School Year 2007-2008.  We look forward to a new year with all the challenges and joys that our ministry brings.  We are honored to be asked to walk with your child/children in this educational journey at St. Mary’s School.  If you have any questions please call or stop by the office.

Parents can deliver registration packets during school hours:

Please bring registration forms to the school office no later than April 9, 2007.

We will open registration to students on the waiting list April 10, 2007.

Please fill out and RETURN the following forms:

1. 2007-2008 Grades K - 8 Tuition Contract & Electronic Funds Agreement (pink).....One per family

2. Student File Information/Medication & Health Policy (green).....One for each student

3. Emergency Contact Card.....One for each student

4. Technology Usage Policy (salmon).....One per student

5. Parent Service Hour Commitment Form (blue).....One per family

6. Shirt/Sweatshirt Order Form (orange).....One per family

7. Financial Assistance Form (lemon yellow).....Available to eligible Families

• School Nurse Letter / Asbestos Hazard Emergency Response Letter (white)

Birth Certificates, Baptismal Certificates and Immunization Records:

Please provide copies of each for new students and those who have received a request letter.

Student Enrichment Program

St. Mary’s does offer an after school and summer enrichment program.  Space is limited to 24 students.   If you are interested in enrolling your child, please contact Siobhann Marron at the school office.

Snow Days

St. Mary’s School will follow the snow day procedures established by the Boise School District.  If Boise Schools are closed for a snow day, St. Mary’s School will also be closed.  Parents should not rely on radio and TV stations to specifically report that St. Mary’s is closed.  Parents need to remember if Boise Schools are closed St. Mary’s will also be closed.
